At about 16 years of age, I was at my heaviest (circa 247). I recall at that time, my doctor telling me I had high blood pressure & cholesterol, to which I thought, "damn, I don't want to be a teenager, & end up having a stroke or heart attack". Ironically, I had read about two high schoolers (athletes, nonetheless) that had heart problems & died, so I thought dieting would be a step in the right direction.

The gym aspect actually came after I'd already lost about 40lbs of the excess weight, as my Aunt bought a short-term membership for me at her local health club. It was that spark that created the habit, I haven't been able to kick since then, LOL.
